📋 Description

• Perform telephonic member outreach and face-to-face encounters to facilitate program enrollment • Conduct departmental assessments and evaluate members holistically to identify needs, health goals, and barriers to wellness • Develop, implement, and revise member-specific plans of care and care manager interventions • Complete documentation according to departmental policy and regulatory standards • Provide targeted health education and proactive condition-management strategies • Communicate with providers and vendors involved in member care • Coordinate safe transitions to the next level of care, including post-discharge instructions, services, and follow-up • Implement strategies to prevent readmission • Collaborate with interdisciplinary care teams to improve member outcomes • Attend and present high-risk members at interdisciplinary rounds • Maintain professional growth through self-directed learning

🎯 Requirements

• Registered Nurse with current unrestricted license in state of residence • May be required to obtain other state licensure in states where Point32Health operates • Understand and follow the provisions of state-specific Nurse Practice Act(s) where Point32Health operates • National certification in Case Management desirable • Bachelor’s degree or relevant equivalent experience • 5 years’ relevant clinical experience • Experience in home care or case management preferred • Proficiency in a second language desirable • Experience in specialty areas a plus • Skill and proficiency in Microsoft applications, technical concepts and principles, and computer software applications • Skilled in assessment, planning, and managing member care • Advanced communication and interpersonal skills • Ability to address multiple complex issues • Ability to organize and prioritize work and member needs • Strong clinical and critical thinking skills • Regard for confidential data and adherence to corporate compliance policy • Ability to make face-to-face visits as needed • Valid driver’s license and vehicle in good working condition • May be required to work additional hours beyond standard work schedule
