📋 Description

• Review each discharged patient’s record for incomplete or missing items • Identify responsible providers and link deficiencies for completion • Monitor the reanalysis queue • Verify chart completion and perform quantitative analysis for inpatient, Skilled Nursing Facility Unit, ambulatory surgery, and GI lab medical records • Review records for missing documents, signatures, and required text • Assign deficiencies to appropriate medical staff • Enter analysis details into the electronic chart-tracking system • Confirm records are analyzed • Provide backup support for prepping, scanning, and indexing • Assist physicians one-on-one with completing medical records • Re-analyze records and update the chart-tracking system • Prepare weekly pre-suspension reports for medical staff leadership • Operate scanners, copiers, fax machines, computers, and phones • Ensure work meets JCAHO, CMS, and Medical Staff Rules and Regulations standards

🎯 Requirements

• High School or GED • Two to five years of relevant experience required • At least one year—preferably two—in an acute care setting with responsibility for documentation requirements • Demonstrated competency in all areas of Medical Records • Strong skills in medical record completion and discharge analysis • Knowledge of medical terminology and anatomy preferred • Experience with electronic patient record systems preferred • Strong written and verbal communication skills • Must work 10 Hour Nights (United States of America)
