• Collect, validate, and clinically quality-review patient-level data for national clinical quality initiatives and clinical data registries • Manage the medical record retrieval process for required data abstraction • Collect complex data elements using established definitions • Ensure integrity of electronic data transmitted to national databases/registries • Prepare and interpret clinical data reports • Work with clinical team members on quality improvement projects • Translate clinical documentation into actionable improvement information • Identify system variances, evaluate potential causes, and support action plans for clinical care improvement • Support leadership teams, subject matter experts, stakeholder groups, and ad hoc teams related to data abstraction and performance improvement • Provide guidance, consultation, and standard work for registry and clinical database design and maintenance • Abstract clinical data from electronic medical records according to guidelines and patient-selection criteria • Develop efficient data collection and management processes and validate data integrity • Upload and submit data files; investigate and correct submission errors • Collaborate with clinical leaders to achieve quality patient care and accurate documentation • Report quality outcomes, performance, and compliance results to clinical stakeholders • Review medical records for deviations or non-compliance, identify trends and patterns, and provide stakeholder feedback • Perform other duties as assigned
• Associate degree required • Preferred education in Healthcare related field, Health Information Management or related field of study • Two (2) years clinical data abstraction, data management or data entry experience preferred • In lieu of Associate degree, five (5) or more years directly related work experience in data abstraction will be considered • Proficient computer skills (word processing, spreadsheets, PowerPoint database, data entry) • Mathematical skills • Knowledge of electronic health records • Working knowledge of quality improvement, clinical data review, and performance improvement methodologies • Ability to effectively communicate opportunities for improvement • Detail oriented • Ability to work independently and adhere to standards/definitions of sponsoring agencies/entities • Communication skills and ability to interact and build relationships within quality improvement teams • Ability to utilize appropriate resources to make decisions regarding data abstraction, interpretation and dissemination • Knowledge of human anatomy, medical terminology, electronic medical record, data abstraction/data entry • If working with Cancer registry, Oncology Data Specialist Certification by the National Cancer Registrars Association (NCRA) is required within one year of employment in the role
