• Support the care navigation team while working directly with unit RN Acute Care Navigators and Social Workers • Communicate with post-acute providers and provide discharge planning and clinical updates • Schedule follow-up appointments with primary care providers and specialists for high-risk patients • Reschedule appointments as needed if discharge is delayed • Coordinate with vendors, physician offices, and clinics for discharge planning • Help ensure smooth transitions of care • Navigate and document properly in the electronic medical record • Perform duties promoting quality patient care, customer service/satisfaction, safety, and organizational principles
• High School Diploma • 2–3 years of clerical experience working in a medical setting • Willingness and ability to adapt to change and support process changes • Positive rapport with patients, families, physicians, and interdisciplinary teams • Exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skills • Ability to work in a self-directed environment with attention to detail and follow-through • Ability to multitask, prioritize tasks, and complete work accurately in a fast-paced environment • Computer literacy and understanding of Microsoft Office products • Ability to navigate the electronic medical record and document properly • Ability to communicate effectively and professionally via phone, email, and in person • Ability to work as a team player with interdisciplinary teams • Working knowledge of medical terminology • Ability to work with people of all social, economic, and cultural backgrounds • Flexibility, open-mindedness, and adaptability to change • Ability to work with minimal supervision
