📋 Description
• Evaluate and manage patients' changing health and service needs • Provide optimum individual care for the patient/family unit using the nursing process and interdisciplinary team members • Collaborate with the patient's Primary Care Physician and other health team members • Create and implement written care plans within documentation standards timelines • Serve as an integral member of the CHAT or ACO team
🎯 Requirements
• Upon hire: Washington Registered Nurse License • Bachelor's Degree in Nursing preferred • 3 years of current experience in a health care environment with home health case management experience preferred
