📋 Description
• Evaluate and manage the changing health and service needs of patients. • Provide optimum individual care for the patient/family unit using the nursing process. • Collaborate with the patient's Primary Care Physician and other health team members to create and implement a written care plan.
🎯 Requirements
• Upon hire: Washington Registered Nurse License • Bachelor's Degree Nursing (preferred) • 3 years Current experience in a health care environment with home health case management experience (preferred)
