• Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team • Supervises Care Management (CM) staff to achieve the goals for the organization • Collaborates directly with inpatient and ambulatory clinicians and care teams
• Washington Registered Nurse License upon hire • 5 years of Clinical nursing, including acute care hospital and/or ambulatory clinic experience • Bachelor’s Degree (Preferred) • 3 years of Case Management experience including care management, care coordination, discharge planning, or population health (Preferred)
