📋 Description

• Execute high-quality community education support for assigned Senior Life Solution programs • Develop strategic referral sources and deliver educational content • Foster partnerships with host hospitals, community organizations, government agencies, and healthcare providers • Organize events, forums, and outreach efforts • Assess and monitor community education goals • Create and maintain a referral source network • Deliver public speaking and tactical advocacy messaging • Cultivate relationships with host hospitals and program team members • Collaborate with the corporate Community Education team on education tasks • Identify community and referral-source needs and provide tools to program teams • Develop expertise and stay informed in senior health • Monitor referral development performance and analyze data to improve outcomes • Train teams on Referral Process Guidelines, hospital integration, and the Collaborative Model • Implement, track, and monitor community education plans • Participate in strategic planning and quality improvement activities • Develop community outreach curriculum ideas and plans • Provide health-related education to communities and community groups • Adhere to safety and infection control guidelines

🎯 Requirements

• Bachelor’s degree in education, marketing, communications, public relations, business administration, or a combination of education and experience • Health care and mental health experience preferred • Demonstrated success in community outreach, education, marketing, or business development • Strong public speaking, relationship-building, and organizational skills • Ability to travel up to 75% of the time • Ability to work with healthcare industry regulations • Ability to travel by car • Ability to perform intermittent physical activity, including standing, walking, bending, kneeling, stooping, and crouching • Ability to talk and hear; close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus • Adherence to safety and infection control guidelines
