📋 Description

• Provide case consultation and collaborate with interdisciplinary care teams. • Conduct case management, including assessing patient needs, coordinating care, and connecting patients with community resources. • Lead group sessions focused on psychoeducation, stress management, mindfulness, and meditation. • Conduct one-on-one patient visits to provide emotional support, stress management techniques, psychoeducation, and coping strategies. • Document patient encounters and coordinating follow-up care to support positive health outcomes.

🎯 Requirements

• LCSW license with licensure to practice in NY, FL, and MI (also open to PA, LA, OH) • 2+ years experience as a LCSW • Bachelor’s and Master’s degrees required • Experience communicating with patients asynchronously preferred • Experience with GI conditions or weight management a plus • Proficiency in using telehealth platforms and electronic health record (EHR) systems • Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease • Experience working with patients in a clinic setting preferred • Demonstrates a willingness to go above and beyond normal responsibilities to provide the best care possible • Excellent interpersonal communication and problem-solving skills • Exceptional organizational skills • Experience and ability to thrive in a fast-paced, high-growth environment • Strong personal computing skills - remote work experience preferred • Passion for and commitment to functional medicine approaches
