📋 Description

• Provide telephonic clinical assessment, nursing triage, health information, and patient education to diverse patient populations • Provide health education to culturally diverse patients recently discharged from inpatient admission • Direct patients to the most appropriate level of care • Coordinate care closely with other team members • Respond quickly and professionally in stressful and emergent situations using critical thinking • Adhere to and participate in the Care Management process • Provide outbound follow-up calls as necessary • Perform other duties as assigned

🎯 Requirements

• High School Diploma – Required • LPN License – Required • Multistate License issued under the NLC (Nurse Licensure Compact) – Required • LPN experience required • Home Health, Med Surg, or Emergency Department experience preferred • Excellent verbal and written communication skills • Excellent organizational skills and ability to complete competing priorities • Advanced computer skills • Must be team oriented • Must be able to work Eastern Time Zone, Monday–Friday, 8-hour shifts starting at 8:30am EST
