📋 Description

• Utilize critical thinking and judgment to collaborate and inform the case management process • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Collaborate with members, providers, and community organizations • Address members’ health care and social determinant needs across the continuum of care • Travel throughout Sangamon, Macon, Logan, and surrounding areas as needed

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Sangamon, Macon, Logan counties • Minimum 3-5 years clinical practical experience • Dedicated workspace free of interruptions • Dependents must have separate care arrangements during work hours • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois
