📋 Description

• Recruit, hire, train, and onboard new team members • Provide direct supervision and performance management for assigned staff • Conduct regular 1-on-1 meetings, performance reviews, and coaching • Identify professional development needs and facilitate training and continuing education • Address and document performance issues • Prepare staff schedules and manage coverage • Maintain compliance with healthcare regulations, including HIPAA and state nursing boards • Ensure timely and accurate documentation and record-keeping • Participate in quality improvement and process optimization initiatives • Track and report key performance indicators and metrics • Coordinate with departments to ensure seamless patient care transitions • Maintain appropriate staffing ratios and coverage • Communicate with patients, families, and interdisciplinary team members • Participate in team meetings and clinical huddles • Serve as liaison between clinical staff and management • Report team performance, concerns, and successes to leadership • Collaborate with community partners and external resources • Coordinate, schedule, and reschedule clinical appointments • Manage referrals to specialists and external clinical services • Follow up with families to reduce gaps in care and no-show rates • Provide clinical updates to referring physicians • Coordinate lab work and documentation requirements with referring providers • Support providers for patients requiring escalation of care • Provide evidence-based education to families regarding chronic condition management • Explain medication regimens, treatment plans, and care goals • Respond to patient and family inquiries via messaging and phone • Initiate and manage medication prior authorization requests • Communicate with pharmacies to resolve dispensing issues and coordinate refills • Track authorization status and escalate delays • Maintain accurate medication records and document pharmacy communications

🎯 Requirements

• Active California Registered Nurse (RN) license in good standing • Associate's, Bachelor's, or Master's degree in Nursing accepted • Experience leading a team of clinical staff • Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems • Strong organizational, communication, and interpersonal skills • Ability to manage competing priorities and work collaboratively in a remote, virtual environment • Comfort working with diverse, medically and socially complex pediatric populations • Excitement about joining a fast-growing startup and iterating on new care models • Reliable internet connection and a dedicated workspace suitable for remote work • Bilingual: English/Spanish • Preferred: Experience in pediatric, chronic care, or care coordination settings • Preferred: Familiarity with prior authorization processes and payer requirements • Preferred: Experience working in underserved or urban environments • Full-time availability, 40 hours per week initially • Availability Monday–Friday, 9 AM–6 PM PST, with a 1-hour daily break • Occasional Saturday coverage
