📋 Description

• Assess patient needs and administer care coordination plans • Develop personalized care plans and facilitate seamless transitions between care settings • Analyze patient data from medical histories, diagnostic test results, and treatment plans • Apply case management and nursing knowledge to organize patient files • Communicate securely through internal platforms, including messaging and video conferences • Conduct utilization reviews to ensure access to appropriate, cost-effective care • Configure case management systems for disease management and utilization review • Track patient progress and manage specific conditions • Coordinate analytics projects and generate reports on key health performance indicators • Design discharge-planning processes for safe, timely hospital-to-home transitions • Develop resource management strategies using community healthcare resources • Advocate for patients and coordinate services across healthcare providers

🎯 Requirements

• 5+ years of relevant work experience • Active and unrestricted Registered Nurse Licensure in state of residence • Working knowledge of problem solving and decision making skills • Working knowledge of medical terminology • Working knowledge of digital literacy skills • Ability to deal tactfully with customers and community • Ability to handle sensitive information ethically and responsibly • Ability to consider the relative costs and benefits of potential actions • Ability to function in a clinical setting with diverse cultural dynamics • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently residing • Associate’s Degree in Nursing (required) • Bachelor’s Degree (preferred)
