📋 Description

• Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications • Support Inpatient Case Management, Healthy Planet, and EpicCare Link • Partner with clinical, operational, and technical teams to analyze workflows • Support system enhancements and evaluate software releases • Resolve application-related issues • Support application integration, system upgrades, testing, and ongoing operational activities • Contribute to Sentara's Population Health growth and strategy • Support improvements in patient outcomes, quality of care, and clinician efficiency

🎯 Requirements

• Bachelor's Degree in Nursing, Health Information Management, Healthcare Administration, Information Technology, or a related field (Required (Preferred) • 5 years of relevant experience may be accepted in lieu of degree • Epic HP (Healthy Planet), IP Clin Doc (Inpatient Clinical Documentation), or Epic Case Management Certification preferred • 3 years of relevant healthcare, clinical informatics, application support, or IT analyst experience with a degree • 5 years of relevant experience in lieu of a degree • Experience in clinical case management, care coordination, or population health programs required • Knowledge of inpatient nursing workflows and healthcare operations • Familiarity with Epic Healthy Planet, population health management tools, or healthcare technology applications preferred • Experience supporting software testing, workflow analysis, system upgrades, or application optimization activities preferred • Strong analytical, problem-solving, communication, and customer service skills • Ability to work effectively in a remote, collaborative team environment
