📋 Description

• Provide high-quality telephonic support for the care enhancement program • Coordinate with Primary Care Providers and healthcare team members to support total healthcare management, appropriate resource utilization, and quality outcomes • Identify participants at risk for increasing acuity and intervene according to disease management guidelines • Review preventive health exam status and initiate appropriate interventions • Contact participants for information sharing, assessment, intervention, and behavioral goal setting • Identify educational needs and facilitate educational opportunities • Review functional status, family support systems, desired care outcomes, and education needs • Develop and facilitate care plans addressing participants’ total healthcare needs • Identify barriers to accessing healthcare services and coordinate with program teams to remove system barriers • Facilitate communication among physicians, providers, health plans, and the disease management team • Coordinate continuity of care for assigned participant panels • Accept warm-transfer calls from non-RN colleagues for assessment or additional participant discussion

🎯 Requirements

• Current RN license appropriate for the state in which the Business Unit is located and able to obtain licensure in other states, as required by state law or contract • High School diploma and a Diploma in Nursing, an Associate’s Degree in nursing or Bachelor’s Degree in nursing with a BSN preferred • Basic knowledge of physiology and chronic disease appropriate for an RN • Minimum of 3 years recent experience in a clinical setting • Must have received the COVID-19 vaccine by date of hire to be considered for employment • Excellent listening skills • Ability to be self-directed, highly organized, and proactive • Participation in orientation and completion of pre- and post-tests • Yearly competency test required for all Sharecare clinicians
