📋 Description

• Provide integrated, hands-on support to patients and physicians to improve patient care quality • Develop personalized relationships with patients and prescribers to improve medication adherence and promote evidence-based care • Review clinician notes and assessments for clinical data needed for prior authorization approvals and care plan recommendations • Develop and refine disease- and drug-specific clinical protocols • Monitor patient adherence using PDC reports, patient assessment data, and mobile platform data • Evaluate EMR information including drug/disease interactions, medication selection, labs/vitals, allergies, prescriptions, renewals, and physician communications • Recommend alternative therapeutic regimens and disease-specific treatments according to guidelines • Monitor therapies for side effects and medication-related issues and recommend changes to prescribers • Perform medication therapy management, document interventions, and assess adherence rates • Conduct pharmacist consultations, motivational interviewing/coaching, drug regimen optimization, medication reconciliation, medication synchronization, and related clinical interventions • Coordinate home nurse visits and transitions between liaison/PSC teams • Monitor productivity and report weekly growth-development statistics • Oversee technicians' daily activities and verify delivery coordination slips, prior authorizations, logs, and care-utilization management • Identify and resolve insurance issues and recognize opportunities for financial assistance • Supervise support-personnel communications in compliance with HIPAA rules and regulations • Maintain current registration and remote pharmacist activity in compliance with state and federal drug laws and DEA regulations • Communicate directly with other health professionals regarding patient care • Train pharmacy staff, students, interns, externs, and healthcare professionals • Manage, organize, and update data using database applications • Support operational improvements, productivity, quality, customer-service standards, research projects, peer review, and process-improvement initiatives • Identify trends, resolve problems, recommend improvements, and implement change • Protect confidential information and maintain professional knowledge through education and publications • Perform other duties as assigned

🎯 Requirements

• Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree • Applicant must be a licensed Pharmacist • North Carolina pharmacist license required by the job title/metadata • Additional required licensure must be obtained within a mutually agreed upon timeframe if needed; continued employment is contingent upon meeting these requirements • 1+ years' experience is strongly preferred but not required • Strong verbal and written communication skills • Deep knowledge of specialty medication pharmacology • Knowledge of local, state, and federal pharmacy regulations • Ability to manage processes and demonstrate strong technical and clinical skills • Computer literacy and proficiency with Microsoft Word, Excel, PowerPoint, Outlook, and Teams • Working knowledge of database and reporting tools • Ability to identify barriers to medication adherence and implement and monitor improvement plans • Ability to motivate and coach patients to improve outcomes and identify therapy goals • Knowledge of complex/chronic disease states and specialty pharmacy therapies preferred • Solid computer skills, internet and web-based platform navigation • Data entry and computer skills will be evaluated during pre-employment assessment
