📋 Description
• Responsible for overall operation of case management and social services activities • Performs concurrent patient assessments related to appropriateness of care • Provides consultations to medical staff and interdisciplinary team members • Documents discharge planning services professionally and collaboratively • Facilitates care conferences with patients and families
🎯 Requirements
• Master’s Degree in Social Work or Counseling • LCSW or LCPC in Illinois • 2 years of experience in providing individual, family and/or group therapy • Experience in addiction disorders/cross addictions • Excellent interpersonal skills • Excellent knowledge of available community resources
