📋 Description

• Provide compassionate, client-centered personal care and supportive services in clients’ homes • Assist with activities of daily living, including bathing, dressing, grooming, toileting, ambulation, and feeding • Assist with prescribed exercises and mobility as trained and permitted by regulation • Provide medication reminders as directed by the Nursing Supervisor • Take and record vital signs and report changes in client condition • Prepare meals and assist with feeding as needed • Perform light housekeeping • Provide companionship and emotional support • Document care provided and report concerns promptly • Use GPS/EVV app and company software for shift clocking • Communicate with the Scheduling Coordinator regarding assignments, availability, and scheduling needs • Follow individualized care plans directed by nursing staff • Adhere to agency policies, procedures, state regulations, HIPAA guidelines, and the agency code of conduct • Attend required training sessions and in-service education

🎯 Requirements

• Current Home Health Aide certification or Certified Nursing Assistant license in Massachusetts • CPR certification (current and valid) • Excellent interpersonal skills and ability to work independently • Reliable transportation and willingness to travel within the service area • Ability to stand, walk, bend, and lift for extended periods • Ability to bend, stoop, reach and move client weight up to 250 pounds • Ability to lift/carry up to 30 pounds • High School Diploma or equivalent (preferred) • One (1) year of experience in a home care setting preferred • Flexible availability including weekdays, weekends, evenings, and holidays depending on client needs and agency requirements
