📋 Description

• Learn and use Solace systems, tools, technology, partners, and expectations • Provide clinical expertise in patient interactions • Identify and prioritize patients’ needs • Maintain a streamlined care continuum • Oversee patient care plans with healthcare advocates • Address social determinants of health, including food resources, transportation access, and support at home • Build relationships and facilitate an advocacy experience for patients • Collaborate with healthcare advocates to help patients achieve health goals • Contribute to developing a new patient care program

🎯 Requirements

• Must reside in Missouri • Applicants must be based in the United States • Must have a DEA license • Must have HAP • Board certification in family medicine, internal medicine, or another relevant specialty • Strong clinical skills • Excellent organizational skills • Ability to balance multiple tasks and work under pressure • Superior communication, listening, and empathy skills • Comfortable working with elderly patients and individuals undergoing Social Determinants of Health • Ability to quickly learn new systems and software • Active license to practice medicine • Active medical license in one or more states • Must be board certified
