Sprinter Health is a hybrid in-home and virtual primary care provider that expands access to proactive, preventive healthcare by combining community-based, W-2 “Sprinters” (trained phlebotomists with medical assistant and community health worker skills) who perform hands-on diagnostics and wellness checks in patients’ homes with a virtual clinical team of physicians, nurses, pharmacists, and care navigators. Their full-stack platform and operations close care gaps, deliver preventive screenings and diagnostics, coordinate medications and referrals, and scale longitudinal primary care and wellness visits for payer and provider partners and patients.

📋 Description

• Review the daily caseload and prepare technology and tools for patient service • Navigate the daily schedule using modern technology • Partner with clinical in-home team members visiting patients’ homes • Build welcoming connections with diverse patients • Collect relevant patient information and perform wellness services • Conduct health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about their health and well-being • Evaluate vital signs and arrange blood draws with Sprinters • Perform necessary tasks to address patient-specific needs • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Prior experience performing wellness visits • Dependability, professionalism, and problem-solving abilities • Commitment to exceptional customer service • Ability to work autonomously while maintaining high-quality standards • Friendly, compassionate, empathetic, and personalized approach to patient care • Willingness to work in a revolutionary environment requiring last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops, mobile devices, and HIPAA-secure messaging apps for charting and care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Ability to pass a national background check and valid clinical license search • Application form asks about Medicare and/or Medicaid Annual Wellness Visit experience, Medicaid enrollment, virtual Family Nurse Practitioner experience, prescriptive authority, and board certification
