Sprinter Health is a hybrid in-home and virtual primary care provider that expands access to proactive, preventive healthcare by combining community-based, W-2 “Sprinters” (trained phlebotomists with medical assistant and community health worker skills) who perform hands-on diagnostics and wellness checks in patients’ homes with a virtual clinical team of physicians, nurses, pharmacists, and care navigators. Their full-stack platform and operations close care gaps, deliver preventive screenings and diagnostics, coordinate medications and referrals, and scale longitudinal primary care and wellness visits for payer and provider partners and patients.

📋 Description

• Review the daily caseload and prepare technology for patient services • Navigate the daily schedule using technology and partner with clinical in-home team members visiting patients’ homes • Establish welcoming connections with diverse patients and collect relevant information • Conduct virtual adult and/or pediatric wellness visits and health assessments • Perform health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about their health and well-being • Work with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation • Use autonomous clinical judgment and maintain high-quality standards

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and think outside the box • Technologically savvy and comfortable using laptops or mobile devices for charting • Comfortable using HIPAA-secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass a national background check and valid clinical license search • Must currently hold prescriptive authority in the state(s) where licensed to practice • Must be a Medicaid-enrolled provider • Virtual experience as a Family Nurse Practitioner requested in the application • Prior Medicare and/or Medicaid Annual Wellness Visit experience requested in the application
