📋 Description

• Review the daily caseload and prepare technology and tools for patient services • Navigate the daily schedule using modern technology • Partner with clinical in-home team members who visit patients’ homes • Build warm connections with diverse patients • Collect relevant patient information and perform various services • Conduct virtual adult and/or pediatric wellness visits • Perform health risk assessments, medication reviews and cognitive screenings • Provide patients with educational information about health and well-being • Evaluate vital signs and arrange blood draws with in-home team members • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in any of the following states: KY, MI, OH, IL • Prior experience performing wellness visits • High levels of professionalism, integrity, accountability, confidentiality, care and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search • Experience with Medicare and/or Medicaid Annual Wellness Visits • Virtual experience as a Family Nurse Practitioner • Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner
