Sprinter Health is a hybrid in-home and virtual primary care provider that expands access to proactive, preventive healthcare by combining community-based, W-2 “Sprinters” (trained phlebotomists with medical assistant and community health worker skills) who perform hands-on diagnostics and wellness checks in patients’ homes with a virtual clinical team of physicians, nurses, pharmacists, and care navigators. Their full-stack platform and operations close care gaps, deliver preventive screenings and diagnostics, coordinate medications and referrals, and scale longitudinal primary care and wellness visits for payer and provider partners and patients.

📋 Description

• Review the daily caseload and prepare technology and tools for patient service • Navigate the daily schedule using modern technology • Partner with clinical in-home team members visiting patients’ homes • Build welcoming connections with diverse patients • Conduct virtual adult and/or pediatric wellness visits • Perform health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about their health and well-being • Evaluate vital signs with Sprinters and arrange blood draws • Carry out tasks addressing patients’ specific needs • Coordinate comprehensive care, including preventive interventions, medication management, specialist referrals, community resources, and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in Michigan • High professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to solve problems creatively in a changing environment • Technologically savvy and comfortable using laptops, mobile devices, charting tools, and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Must pass a national background check and valid clinical license search • Prior experience performing wellness visits
