📋 Description

• Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone • Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics • Deliver high-quality, cost-effective care addressing acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers • Perform comprehensive assessments and accurately document encounters in the EMR in compliance with CMS requirements • Apply Curana’s clinical protocols and practice guidelines to support safe, effective treatment in place whenever possible • Participate in mandatory education and training to remain current with standards of care • Provide telephonic and telehealth care to aging residents across multiple states, including evaluation of acute changes, falls, and controlled substance visits • Manage multiple calls independently during evenings, nights, weekends, and holidays

🎯 Requirements

• Master's Degree as a Nurse Practitioner • Active, unrestricted licensure in Texas and Louisiana and/or Tennessee, or another approved state • Nurse Practitioner national certification as ANP, FNP, or GNP • Ability to obtain DEA licensure / Prescriptive Authority • Background in acute and chronic disease management • Clinical background in adult, family, or geriatrics • 3+ years of experience as a Nurse Practitioner • Ability to gain a collaborative practice agreement, if applicable in applicable state(s) • Ability to work scheduled shifts in accordance with scheduling policies • Proficient computer skills, including documenting medical information with written and electronic medical records • Overnight and holiday availability required; two holidays per year required • Every other weekend availability for 12-hour Saturday and Sunday shifts • Experience in a nursing home or acute care facility with seniors preferred • Understanding of geriatrics, chronic illness, acute disease management, advanced illness, and end-of-life discussions preferred • Ability to develop and maintain positive customer relationships • Adaptability to change • Company is unable to provide visa sponsorship (H1B or otherwise)
