📋 Description

• Provide comprehensive biopsychosocial and standardized assessments for potential program enrollment • Develop individualized support plans with patients, including goals • Provide community service resources and make referrals • Provide enrollment and intake services • Respond to telephone inquiries about mental health care and related services • Conduct safety assessments and planning via telephone or video • Partner with patients to create self-directed, sustainable changes aligned with their values • Collaborate with multidisciplinary team members to develop patient support goals • Participate in multidisciplinary patient care conferences as needed • Document interviews, consultations, referrals, contacts, and supportive services • Complete documentation and reporting according to departmental standards and federal and state regulations • Work in a constant state of alertness and safe manner • Perform other duties as assigned

🎯 Requirements

• Must live in one of the following remote states: Alabama, Florida, Georgia, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Michigan, Missouri, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, Utah, West Virginia, or Wisconsin • LCSW or LPC preferred/required skills listed • Experience with Epic • Strong communication skills • Teamwork skills • Crisis intervention skills • Master's degree in Behavioral Health or education equivalency for licensing • No experience required • Must hold an applicable state professional license/certification for the state of work location, including eligible LCSW, LPC/LCPC, LMFT, LADC, LBP, LMSW, LADC-MH, MFT-TL, APSW, or LPC-IT credentials as listed
