• Assess the quality of care and services of healthcare facilities/providers through onsite observations, interviews, and record reviews • Identify deficient practices and relate findings to federal and state statutory and regulatory requirements • Collaborate with other surveyors to evaluate compliance with quality care, quality of life, and structural requirements • Write deficiency statements according to the CMS Principles of Documentation • Complete reports and summaries according to CMS and state guidelines and timelines • Use computer programs and applications proficiently • Pass the CMS Federal Qualification Test (SMQT) within six to twelve months of hire
• Possession of, or eligibility for, licensure as a Registered Nurse (RN) in Vermont, or eligibility to practice in Vermont via a multi-state license • Four (4) years or more of professional nursing experience • Must maintain Vermont licensure or a multi-state Registered Nurse license as a condition of employment • Must pass the Surveyor Minimum Qualifications Test (SMQT) certification within one year of hire • Must pass applicable background investigation and required checks, including criminal record, abuse registry, OIG exclusion database, and motor vehicle record checks • Must be able to travel significantly throughout Vermont • Must be able to spend nights in hotels several times a week/month • Must have a reliable means of transportation • Must demonstrate proficiency using computer programs and applications • Evening and weekend work is required • Bachelor of Science in Nursing (BSN) preferred
