📋 Description

• Manage the intake process for new patients, including receiving referrals from healthcare providers, insurance verification, and scheduling initial assessments • Serve as the primary point of contact for patients, families, healthcare professionals, and referral sources regarding Home Health and/or Hospice services • Ensure accurate and timely documentation of patient information, including medical history, assessments, care plans, and communication with other healthcare providers • Collaborate with interdisciplinary team members to develop individualized care plans based on patients' needs, goals, and preferences • Coordinate services and resources needed to support patients' care plans, such as medical equipment, supplies, therapy services, and community resources • Provide education to patients and families about their condition, treatment options, self-care techniques, and available support services • Monitor patients' progress and response to treatment, reassessing needs as necessary, and adjusting care plans accordingly • Coordinate transitions between care settings, such as hospital discharge to Home Health and/or Hospice services, ensuring continuity of care and smooth transitions • Participate in quality assurance activities, such as audits, reviews, and performance improvement initiatives, to ensure compliance with regulatory standards and quality outcomes • Collaborate with healthcare professionals, including physicians, nurses, therapists, and social workers, to ensure coordinated and comprehensive care for patients

🎯 Requirements

• Associates Degree or Certification in Nursing • Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC) • Two (2) years of Home Health and/or Hospice experience (preferred) • Two (2) years of experience in utilization review/chart audit (preferred) • Bachelor’s degree in nursing (preferred)
