📋 Description

• Provide high-quality home health care to patients in their preferred home settings • Align all activities with Elara Caring’s vision and Home Health leadership • Maintain patient and staff privacy and confidentiality under HIPAA • Assess assigned home health patient caseloads and families for physical, psychosocial, and environmental needs • Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating treatment plans • Collect physical, psychological, social, and spiritual patient data and incorporate it into plans of care • Conduct initial evaluations and reevaluate nursing needs during visits • Make follow-up visits based on patient, family, and facility needs • Communicate significant findings, problems, health-condition changes, environmental issues, and unsafe facility conditions to relevant clinical and care personnel • Revise plans of care in response to patient-care issues and notify the Clinical Supervisor, physician, facility, and team members • Report to the Clinical Manager or Clinical Supervisor • Travel within the branch/office coverage area and between patient homes and assigned work locations

🎯 Requirements

• Current, unrestricted state RN license • 1+ year of experience in a clinical care setting as a nurse • Ability and willingness to travel within the branch/office coverage area • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment • Valid driver's license with an acceptable driving record, or reliable transportation between patient homes and assigned work locations • Ability and willingness to travel 50% • Associate’s Degree or Bachelor’s Degree in Nursing is preferred • Previous Home Health or Hospice experience is preferred • Previous experience with HomeCare HomeBase is preferred • Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
