• Provide clinical staffing support across high-need Well Care Home Health regions • Provide direct nursing services and case management support during vacancies, census fluctuations, and other staffing needs • Assess patients and families/caregivers and complete initial, comprehensive, ongoing, reassessment, and recertification activities • Develop, implement, and update individualized plans of care with physicians, patients, families/caregivers, and interdisciplinary teams • Coordinate care and supervise assigned aides, LPNs, and delegated services • Communicate patient needs, changes in condition, treatment response, orders, safety issues, and care transitions to physicians and agency leadership • Maintain caseloads, visit responsibilities, documentation, clinical notes, and required forms • Travel across assigned locations and counties to provide temporary coverage for PTO, call-outs, FMLA, and uncovered territories • Participate in interdisciplinary conferences, performance improvement, education, and care coordination • Promote patient satisfaction, agency standards, confidentiality, ethics, safety, and regulatory compliance
• Graduate of an accredited or approved school of nursing through an AD, Diploma, or BSN program • Current license to practice professional nursing in the state where care is provided • Multi-state licensure or ability to obtain licensure for assigned service areas may be required • Current CPR certification required • Minimum of two years of RN experience required • Home Health experience required • Ability to work independently and adapt to multiple clinical environments • Ability to manage patient care responsibilities and coordinate interdisciplinary care • Hand/eye coordination for injections, computer and electronic medical record systems, patient care equipment, and clinical procedures • Must communicate and be literate in English • Ability to manipulate patient care equipment and transfer and guard patients • Ability to transfer or maneuver objects weighing at least 50 pounds • Ability to assess patient conditions, formulate care plans, select interventions, evaluate responses, and teach patients • Ability to visually assess patients, auscultate sounds, hear alarms, and communicate effectively • Variable Monday-Friday hours, weekends, holidays, and on-call participation as needed • Valid driver's license, operational vehicle, and ability to travel across assigned Home Health locations • Ability to serve adult and geriatric populations • Ability to work in environments involving bloodborne pathogens, body fluids, infectious diseases, needle puncture risks, animals, traffic, weather extremes, and potential safety threats
