• Remotely monitor and manage patients with heart failure. • Track and trend vital signs. • Virtually assess patients. • Document patient interactions and interventions. • Work with the clinical care team to coordinate care. • Triage patients using protocols and escalate to the appropriate level of care. • Provide virtual nursing assessments of symptoms, medication adherence, and disease progression. • Educate patients and caregivers on heart failure medications, adherence, and potential side effects. • Monitor vital signs and survey responses, identify changes in condition, and document findings. • Answer, direct, and follow up on incoming calls. • Recognize urgent or emergent situations, seek medical provider assistance, and initiate appropriate emergency interventions as directed.
• Bachelor's Degree in Nursing. • Minimum of 3 years experience in cardiology, heart failure management, or critical care nursing. • Strong background in heart failure patient management, including knowledge of GDMT (Guideline-Directed Medical Therapy). • Current license to practice as a Registered Professional Nurse in a compact state and the state of California; a California license is required. • Experience with patient phone triage in-office or via telemedicine. • Ability to learn and proficiently use multiple EMRs and communication applications, including Google products. • Comfort with virtual use of episodic vital sign monitoring devices. • Ability to work from home in a HIPAA compliant environment is required. • Some holidays may be required. • Advanced certification (CCRN, CHFN, RN-BC) highly preferred. • Bilingual English and Spanish fluency highly preferred.
