📋 Description

• Provide comprehensive, remote clinical support to primary care patients and providers • Deliver telephonic and electronic care using Epic and My Health Online • Assess patient symptoms and concerns through telephone encounters • Manage electronic in-basket messages and coordinate care across settings • Facilitate prescription renewal workflows in collaboration with licensed providers • Identify appropriate hospital and community resources • Conduct nursing follow-up to support continuity of care

🎯 Requirements

• Graduate of an accredited school of nursing • RN-Registered Nurse of California • RN-Registered Nurse in State of Residence • BLS-Basic Life Support Healthcare Provider • 2 years recent relevant experience preferred • 3 years experience of practical nursing in hospital, clinic, urgent care, or emergency room/department preferred • 3 years experience with several specialties and subspecialties preferred • Advanced knowledge of professional nursing practice • Exceptional clinical assessment skills • Proficiency in Epic electronic health record (EHR) workflows
