📋 Description

• Serve as the initial point of contact for patients and caregivers • Build rapport and establish trusting relationships with patients and families • Make outbound calls to educate patients about Sylvan Health programs and assist with enrollment • Schedule initial and follow-up telehealth visits with Registered Dietitians • Support patients with scheduling, telehealth visits, and general insurance questions • Send follow-up communications via phone, SMS, email, and patient portal messaging • Support inbound patient communications, appointment adjustments, and visit transfers • Deliver empathetic customer service during patient interactions • Collaborate with Registered Dietitians and other care team members • Document patient interactions accurately in EMR and CRM systems • Resolve patient and caregiver questions professionally and efficiently • Navigate multiple systems and workflows simultaneously

🎯 Requirements

• Legally authorized to work in the U.S. • 2+ years of experience in a patient-facing or healthcare communication role, such as care coordination, patient scheduling, or healthcare customer support • Strong interpersonal and communication skills for connecting with people from diverse backgrounds and varying levels of health literacy • Ability to manage sensitive, complex, or fast-moving situations in real time • Working knowledge of the U.S. healthcare system, including provider visit types, care pathways, basic insurance concepts, and patient navigation workflows • Ability to communicate clearly and confidently across phone, SMS, and email • Strong organizational skills and attention to detail • Ability to adapt to evolving workflows and operational processes • Ability to work efficiently across multiple software systems simultaneously • Previous nutrition or Medical Nutrition Therapy experience or knowledge is helpful but not required • Bachelor's degree preferred, but not required
