📋 Description

• Provide telemedicine care to patients throughout Indiana • Treat patients of both biological sexes • Provide comprehensive hormone management • Deliver medical weight loss care • Provide preventive healthcare and personalized hormone optimization • Conduct clinical assessments and make medical decisions • Build patient relationships and communicate with patients • Complete hiring and credentialing promptly • Begin employment with minimal delay or no need of transfer

🎯 Requirements

• Active Indiana Family Nurse Practitioner (FNP) license with Indiana APRN licensure (Family specialty only) or an active Indiana Physician Assistant license under your primary taxonomy NPI • Current Indiana CSR and FULL DEA registration required at the time of application; no company reimbursement is provided • Current National Provider Identifier (NPI) number • Current board certification applicable to your profession • NPI number and board certification information must be disclosed during the application process • No current or pending malpractice lawsuits • Strong clinical assessment and medical decision-making skills • Exceptional patient communication and relationship-building abilities • Experience or strong interest in hormone replacement therapy, women's health, preventive medicine, or medical weight management preferred • Bilingual English/Spanish highly preferred • Candidates must possess an active DEA at the time of application • License must be fully unencumbered • NPI active, board certification active with proof, and CSR/DEA active for Indiana • Must be able to practice independently • Full-time availability, Monday through Friday • Patients are scheduled between 8:00 AM and 5:00 PM Eastern Time
