📋 Description
• Provide 1:1 coaching to empower members to manage their chronic conditions • Assess and identify individual member’s needs to drive behavior changes • Follow scheduling best practices to meet coaching utilization • Actively participate in process improvement and provide mentoring support to other coaches
🎯 Requirements
• Minimum of a bachelor’s degree • Must possess and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES) • 3 years of professional experience in the healthcare field • Strong background knowledge behind the science and latest treatment options for managing diabetes • Ability to interpret and incorporate the ADCES 7 DSME/T education model into practice
