📋 Description

• Serve as the primary liaison and coach for nursing homes, hospitals, and outpatient practices • Identify areas for improving patient care, safety, and outcomes • Implement improvement activities and achieve healthcare goals • Collaborate with providers and practices to drive measurable improvements in patient outcomes • Partner with local, state, and national organizations to connect providers and patients with relevant initiatives and learning opportunities • Analyze regional, state, and provider-level quality data • Facilitate learning collaboratives • Provide technical assistance to nursing homes, hospitals, and outpatient practices implementing evidence-based practices • Engage partners and providers across the healthcare continuum • Ensure alignment with state and federal healthcare quality objectives

🎯 Requirements

• Bachelor’s degree in public administration, public policy, public health, or a related field, required, or equivalent experience (10 years healthcare experience, associate’s degree with 5+ years’ experience, etc.) • Healthcare quality improvement experience required • In-depth knowledge of quality improvement principles, practices, and methodologies used in healthcare settings • Strong interpersonal and communication skills • Ability to build productive relationships with C-suite leaders, clinicians, administrators, and other health professionals • Familiarity with state and federal regulations impacting healthcare facilities, including CMS quality measures, survey, and accreditation standards • Ability to coach and facilitate improvement activities with providers and partners • Active Enterprise User Administration (EUA) ID required, including establishing an ICT account and obtaining a EUA ID • If no active EUA ID is held, completion of required training and obtaining the EUA ID is required; failure to obtain it within the timeframe may result in termination • Preferred: nursing home/long-term care/hospital and/or outpatient practice setting experience • Preferred: behavioral health experience • Preferred: master’s degree in public health, quality improvement science, health informatics, or related field • Preferred: licensed RN, LPN, MSW, or CPHQ • Preferred: experience working with multidisciplinary teams including physicians, nurses, pharmacists, and administrators • Preferred: project timeline and deadline management, and detailed written reports • Preferred: translating complex data into actionable insights and narratives for multidisciplinary clinical and non-clinical audiences • Telligen will not provide sponsorship for work authorization now or in the future
