📋 Description

• Contact patients and providers to address gaps in care, primarily through phone outreach • Document all patient and provider interactions and outcomes in the call tracking system • Gather patient encounter details, including allergies, medication history, medical conditions, and other relevant information • Contact patients to reconcile medication lists and collect information supporting care • Coordinate with providers and interdisciplinary teams to support medication management services • Assist pharmacists with related clinical and administrative tasks

🎯 Requirements

• Pharmacy Technician National certification is required • Experience with Medication Therapy Management is preferred • Experience with prescription data entry is preferred • Ability to communicate effectively by email and phone • Excellent interpersonal skills and a professional communication style • Experience with Google G-Suite is preferred • Monday–Friday availability between 9a–8p EST • Home internet through cable broadband or fiber optic service with at least 10Mbps download/5Mbps upload when working from home
