📋 Description

• Answer inbound calls to assist members with completing assessments • Recommend service plans based on member needs and capabilities • Make outbound follow-up calls to members • Help members and families develop their capacity to access community resources • Support access to homecare, meals, transportation, errands, home modification, pest control, and social support services • Participate in coordinating and scheduling services • Navigate the product platform and recommend service offerings • Support completion of member orders and benefits planning, organization, and execution • Obtain member testimonial submissions from satisfied members • Meet call, quality assurance, after-call work, member satisfaction, attendance, and adherence metrics

🎯 Requirements

• Passion for improving health outcomes through home and community-based services • Computer proficiency and knowledge of clerical functions • Interpersonal and telephonic communication skills • Excellent written and verbal communication skills • Proven experience answering inbound calls • Ability to work with multidisciplinary teams • Excellent time-management and organizational skills • Ability to work in a fast-paced startup environment • Must be 18 years of age or older • High School Diploma preferred • Experience in public/community health or a related field preferred • Experience in social services, care coordination, healthcare navigation, public health, community organizing, or volunteer work • Reliable internet connection • Designated work environment conducive to professional phone calls and sensitive data • Ability to remain at the designated workstation for the duration of the workday • Ability to constantly operate a computer, phone, and VoIP equipment • Ability to communicate and exchange accurate information • Ability to observe details at close range on a computer screen • Ability to work Monday-Friday, 2:30 PM–11:00 PM EST
