• Serve as a consistent, trusted point of contact for TAY and kinship families through phone, text, email, and digital platforms • Build and sustain relationships with California community partners for warm referrals and collaborative problem-solving • Deploy micro-cash grants as engagement and stabilization tools and capture structured data on family need • Activate crisis support protocols and coordinate timely, stabilizing care through ConnectMe and relevant partners • Conduct comprehensive, Motivational Interviewing-informed needs assessments using ConnectMe’s smart intake tools • Recognize change talk and adjust navigation strategies to individuals’ readiness • Identify resource gaps and systemic barriers to inform strategy, advocacy, and resource development • Collect and document user feedback and satisfaction data • Manage an active caseload across multiple stages of the care journey • Co-develop and track individualized action plans in ConnectMe • Guide individuals through transitions to community partners, service providers, and government programs • Help individuals navigate complex systems, applications, eligibility requirements, and bureaucratic processes • Match individuals with culturally responsive services using ConnectMe’s technology-assisted directory • Support individuals facing digital access or literacy barriers and flag accessibility gaps • Implement and scale ConnectMe tools, including technology-assisted intake, automated follow-up, chatbots, and digital referral workflows • Document frontline insights to improve the platform and practice model • Contribute lived-experience and human-centered design perspectives to product and innovation work • Codify technology-enabled practices and capture lived-experience data through case notes • Enter data accurately and on time while maintaining privacy and ethical standards • Participate in team debriefs, learning sessions, case consultations, and organizational deliverables • Collaborate with Innovation and Tech on systemic barriers, policy insights, and advocacy models
• 3+ years of experience accessing and navigating child welfare systems and/or other social service systems • Ideally 2+ years of professional experience in direct service, case management, or navigation • Trained or experienced in Motivational Interviewing • Ability to practice Unconditional Positive Regard, cultural humility, and sensitivity to lived experiences • Systems navigation experience with complex, fragmented systems • Personal or professional experience with child welfare systems or other social impact work, or strong commitment to learning about these systems • Commitment to centering the voices and experiences of people with lived experience • Solutions-oriented mindset and ability to adapt in a fast-growing, evolving organization • Experience with data entry and documentation in case management, CRM, or tracking systems • Ability and willingness to learn and adapt digital tools • Familiarity with California’s child welfare system, county structures, or specific state programs • Bachelor’s Degree in social work, counseling, human services, or a related field is required • Relevant lived experience and demonstrated professional competency may be considered in lieu of or in addition to formal education • Motivational Interviewing training is expected within the first 90 days of employment • Spanish bilingual ability, written and verbal, is preferred
