📋 Description

• Provide complex care coordination and monitoring for members with primary intellectual or developmental disabilities and possible secondary mental health, substance use, or complex medical needs • Conduct assessments, monitoring, and complex coordination of care while ensuring adherence to waiver requirements • Assist with integrated care management through multidisciplinary collaboration and referrals • Use person-centered planning methods to gather information and address individual member needs • Educate and support members regarding their rights and responsibilities • Complete Discovery activities, including information gathering and assessment • Inform members and legally responsible people about Medicaid, state-funded, natural, and community services and available service options • Assist members and legally responsible people in choosing service providers objectively

🎯 Requirements

• Must meet QP status • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing with four years of full-time MH/DD/SA experience; OR master's degree in a human service field with one year of full-time MH/DD/SA experience; OR bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR bachelor's degree in another field with four years of full-time MH/DD/SA experience • Substance abuse professionals must meet the specified supervised alcoholism and drug abuse counseling experience requirements • For members with LTSS needs, minimum two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • Must have a valid driver's license • Must reside in Trillium's North Central Region: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, or Wilson counties • Must be able to travel within the catchment as required • Must pass a drug test as a condition of employment
