• Monitor and oversee the activities of the Nurse Team to ensure the needs of members, families, and stakeholders are addressed appropriately, expeditiously, and in a courteous manner. • Identify member needs and escalate appropriately to the Nurse Manager, Director, or other appropriate Trillium staff. • Monitor and review data to ensure performance measures and benchmarks are consistently met. • Provide education to staff to ensure performance improvement and to maintain adherence and compliance with accreditation standards. • Provide direct supervision and clinical guidance to nursing staff.
• Three (3) years of experience providing care management, case management, or care coordination to BH/MH/SU/IDD population and a minimum of three (3) years as an RN. • Licensed in North Carolina by the State Board of Nursing as a Registered Nurse (RN). • Must have a valid driver’s license. • Must reside within North Carolina. • Must be able to travel within catchment as required. • Experience working with acute care discharge planning and/or utilization management, including behavioral health, mental health, and physical health care needs (preferred). • Experience coordinating physical healthcare needs (preferred). • A minimum of one (1) year of experience providing direct supervision of staff (2 or more employees) that provide BH/MH/SU and/or IDD services (preferred).
