• Provide direct, patient-facing care to children, teens, and families within the Collaborative Care Model • Spend approximately 75% of time working directly with patients and families • Conduct intake assessments and ongoing follow-up sessions via telehealth • Deliver brief, evidence-based interventions including CBT skills, motivational interviewing, behavioral activation, and parent coaching • Provide in-session education and actionable recommendations to families • Track patient progress using validated screening tools • Support school accommodations, social determinants of health needs, and coordination with OT/PT/ST and other specialty services • Collaborate with pediatricians, psychiatric consultants, care coordinators, and a multidisciplinary care team • Participate in weekly registry review meetings to guide care decisions • Coordinate with board-certified pediatric psychiatrists regarding medication recommendations, patient progress, and care plans • Document all care in the Healthie EHR • Track outcomes and progress using structured workflows • Manage scheduling, follow-ups, and patient engagement • Apply standardized protocols for no-shows, cancellations, and safety • Deliver structured, goal-oriented sessions and communicate proactively with the care team
• Fluency in Spanish is required • Active, unrestricted clinical licensure as an LCSW, LPC, and/or LMFT • Active licensure in one or more of California, Connecticut, Florida, Maryland, Tennessee, Texas, or Virginia on or before the start date • 3+ years of experience OR a minimum of 3,000 supervised hours • Experience working with children, adolescents, and families • Flexibility with hours/availability, including 2 evenings per week and 2–4 weekend days per month • Experience and training in evidence-based modalities such as CBT, MI, and BA • Comfort working in a telehealth environment • Strong communication, organization, and documentation skills • Ability to deliver care with a diversity, equity, and inclusion (DEI) and trauma-informed lens • Experience with or familiarity with pediatric primary care, collaborative care (CoCM), school-based or community mental health settings, measurement-based care, or brief structured interventions is nice to have
