📋 Description

• Administer assessments and screening tools throughout the care timeline • Interpret assessment responses and prioritize member needs • Develop individualized care plans within the multidisciplinary care team • Lead internal and joint rounds to coordinate and optimize the member experience • Use Tuesday Health electronic systems, tools, and technology to deliver coordinated care • Build strong, trusting relationships with members through listening and empathy • Serve as the quarterback of the multidisciplinary team for members with serious illness • Deliver comprehensive, person-centered care to individuals with serious illness • Drive collaboration and transform the member experience

🎯 Requirements

• Active and unrestricted Registered Nurse license in Massachusetts without any board action • Experience in a clinical/medical setting preferred • Experience in a multidisciplinary care model working across network providers, health plan care management support, and employed clinicians • Strong communication and organization skills • Ability to multitask and work under pressure without sacrificing a positive member experience • Ability to know when to escalate and act professionally in an emergent situation • Strong technical skills to navigate documentation systems and tools • Ability to learn new systems as the company grows and evolves • Strong written skills • Ability to drive during daytime, nighttime, or inclement weather • Valid driver's license with safe driving record • State minimum automobile insurance coverage • Comfort with change and ambiguity
