📋 Description

• Conduct Comprehensive Health Assessments through telehealth visits • Review medical history, medications, preventive care needs, and relevant member information • Document HCC risk adjustment findings accurately during visits • Identify and help close HEDIS quality measure care gaps • Use ICD-10 and CPT II codes to support complete, compliant documentation • Deliver clear care plans and follow-up guidance to members • Complete timely documentation supporting quality programs and value-based care workflows • Work within structured processes for consistent visit flow and documentation quality • Expand access to preventive and value-based care for Medicare and Medicaid members • Support accurate risk adjustment documentation and improved quality outcomes

🎯 Requirements

• Active Nurse Practitioner licenses in at least 2 of the eligible states: PA, WA, IA, KS, NJ, MT, WV, IN, WI, TX, TN, AZ, or MO • At least 1 year of Family Medicine or Internal Medicine experience • Experience caring for Medicare and Medicaid populations • Familiarity with HEDIS and risk adjustment workflows • Medicare and Medicaid provider enrollment • Ability to work at least 24 hours per week • Comfortable delivering care virtually and documenting thoroughly in a structured workflow • Experience with ICD-10 and CPT II coding • Value-based care experience, Annual Wellness Visit experience, and HCC risk adjustment documentation experience are nice-to-haves
