📋 Description
• Oversee the MFP Transition Coordination process for individuals transitioning from Skilled Nursing Facilities • Supervise Transition Coordinators • Ensure effective implementation of transition services • Maintain communication with internal staff, external partners, and community resources
🎯 Requirements
• A Bachelor’s degree in public service administration, psychology, sociology, or social work or a human service programmatic field • Four years of experience in directly related human services program OR administrative or consultative experience in a human service program • Experience working with children, adolescent, and adults with MHSU and IDD diagnoses • Licensure/Certification Requirements: Must have a valid NC driver’s license
