📋 Description

• Provide approximately one hour of professional supervision per week and consultation as needed to the ADH Case Manager • Provide professional guidance related to psychosocial assessments, care plans, client-centered goals, interventions, referrals, and follow-up • Consult on complex, high-risk, or challenging client and family situations and recommend appropriate interventions • Review social work documentation and practices for accuracy, quality, confidentiality, and regulatory compliance • Support coordination of social work services with clients, families, caregivers, healthcare providers, community agencies, and interdisciplinary team members • Provide guidance on community resources, healthcare services, social service agencies, benefits, and referrals • Support case management and care coordination for older adults, individuals with disabilities, and other vulnerable populations • Provide guidance on Massachusetts Adult Day Health, Department of Public Health, MassHealth, and other regulatory requirements • Support complete documentation, regulatory compliance, and audit readiness • Identify and communicate psychosocial concerns, safety risks, compliance issues, and recommendations to the ADH Program Director and interdisciplinary team • Promote ethical practice, client advocacy, confidentiality, dignity, and culturally responsive care • Serve as a professional resource regarding psychosocial needs, social determinants of health, community resources, and care coordination • Collaborate with nursing, medical, rehabilitation, behavioral health, case management, and other interdisciplinary team members • Maintain professional knowledge of Massachusetts regulations, healthcare programs, community resources, and social work practices

🎯 Requirements

• Licensed Clinical Social Worker (LICSW) required • Demonstrated knowledge of professional social work practices, including psychosocial assessment, care planning, case management, referrals, advocacy, and client support • Knowledge of community-based healthcare and social service systems • Knowledge of Massachusetts Adult Day Health requirements and applicable healthcare and social service regulations • Strong understanding of confidentiality, professional ethics, documentation standards, and client rights • Excellent written and verbal communication skills • Strong interpersonal, organizational, critical-thinking, and problem-solving skills • Ability to provide constructive professional feedback and guidance to Case Management staff • Ability to exercise sound professional judgment when addressing complex client and family situations • Ability to maintain confidentiality and handle sensitive client information appropriately • Demonstrated commitment to compassionate, person-centered, trauma-informed, and culturally responsive care • Ability to work collaboratively with clients, families, caregivers, providers, community agencies, and interdisciplinary team members • Basic computer proficiency and ability to review and evaluate electronic health records and documentation • Ability to sit for extended periods while using a computer, monitor, keyboard, and mouse • Ability to visually review electronic and written documentation • Ability to perform the essential functions of the position with or without reasonable accommodation • Master of Social Work (MSW) degree preferred • Experience working with older adults, individuals with disabilities, medically complex individuals, or other vulnerable populations preferred • Previous experience in social work supervision, clinical consultation, case management, or professional oversight preferred • Experience or familiarity with Adult Day Health (ADH) programs, Massachusetts Department of Public Health (DPH) requirements, MassHealth Senior Care Options (SCO), Aging Services Access Points (ASAPs), community-based healthcare, long-term services and supports (LTSS), care coordination and interdisciplinary team models, older adults and individuals with disabilities, and community resources and social service systems
