📋 Description

• Develop and implement the overall clinical development strategy, program development, and care management service delivery design aligned with organizational business goals and objectives • Create, manage, and operationalize clinical strategy workflows for acute and post-acute facilities • Lead, manage, and scale a clinical and operational multi-disciplinary team • Coordinate patient care with acute and post-acute providers and physician groups • Develop clinical, operational, provider, and patient outreach and care protocols • Partner with internal teams to drive best practices with clients and ensure success of risk-based programs • Provide feedback and input to solution teams for workflow and outcome enhancements • Develop and lead clinical and operational presentations for current clients and prospects • Identify and triage rising-risk patients, reduce barriers to home discharge, improve post-acute patient progression, and reduce readmission risk • Ensure regional leaders and teams are accountable for care coordination and clinical, financial, and patient experience outcomes • Build partnerships with acute and post-acute providers and review performance and patient outcomes • Partner with analytics teams to measure clinical and financial outcomes and performance benchmarks • Conduct clinical development risk assessments and implement risk management plans • Facilitate communication and coordination across functional areas • Develop clinical documentation strategies as needed • Perform other job duties as assigned

🎯 Requirements

• Bachelor’s Degree or equivalent work experience • At least 10-12 years of relevant work experience • At least 5-6 years of relevant management work experience • Willing to travel up to 40% based on business needs • Willing to work additional or irregular hours as needed • Must work in accordance with applicable security policies and procedures to safeguard company and client information • Must be able to sit and view a computer screen for extended periods • Applicants for U.S.-based positions with WellSky must be legally authorized to work in the United States • Certain client-facing positions may be required to comply with applicable requirements, such as immunizations and occupational health mandates
