📋 Description

• Provide support to patients and health care providers regarding Patient Support services, diseases, and therapies • Assist patients and caregivers with assessments related to Patient Support services • Liaise between patients, families, and health care providers regarding medication access • Address inquiries about program services, disease states, and products • Provide ongoing disease-state and product support to promote patient adherence • Refer patients to healthcare providers for clinical questions beyond program materials • Document patient information and interactions in electronic patient records • Conduct ongoing file reviews and update patient journeys • Follow case-management protocols, including data collection, follow-up, therapy disruption/discontinuation monitoring, adverse-event reporting, and liaison with primary healthcare providers • Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternatives • Coordinate and track patient services according to KPIs to expedite therapy access • Participate in specialized therapy training • Attend pharmaceutical meetings, patient meetings, conferences, and work with patient associations and advocacy groups • Coordinate with healthcare providers, pharmacies, and infusion/injection clinics on treatment logistics and issue resolution • Maintain communication with the manager, team, and client regarding patient cases • Assist with training new Nurse Case Managers and provide feedback on protocols, databases, processes, and interdepartmental processes • Report adverse events and severe adverse events according to program procedures • Perform other assigned duties and tasks • May be required to provide standby/on-call coverage

🎯 Requirements

• A Diploma in health care or a related field; Bachelor’s degree preferred • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields • Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry • Fluency in French and/or English may be required depending on program and geographic location • Experience with healthcare and case management • Ability to solve problems and work independently and as part of a team • Strong analytical and problem-solving skills • Ability to work effectively across functions and within a small team • Ability to consistently deliver high-quality professional work in a dynamic environment • Team leadership, management, motivation, and training abilities • Experience delegating and assigning tasks within a team • Logical, clear, conceptual, creative, and innovative thinking • Ability to be on standby/on call as required
