📋 Description

• Conduct comprehensive clinical assessments integrating medical, behavioral health, and psychosocial needs • Develop, implement, and monitor individualized care plans with members and providers • Coordinate care with medical and behavioral health professionals • Provide crisis intervention, timely referrals, and follow-up support when clinically indicated • Use motivational interviewing and coaching to engage members and promote sustainable health behavior changes • Identify quality of care concerns and escalate appropriately • Maintain accurate, timely documentation in compliance with regulatory and accreditation standards

🎯 Requirements

• Active, unrestricted independent clinical license in your state: LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, or LMHC • 3+ years of post-master’s direct clinical experience • 3+ years of strong knowledge of mental health and substance use disorders • 3+ years of experience with crisis intervention and motivational interviewing • 3+ years of case management experience • Ability to multitask in a fast-paced, telephonic work environment • Strong technical skills • Master’s degree in social work or counseling required
