• Review referral requests to ensure they meet established criteria • Ensure required data is collected and entered • Attend meetings with referral sources as required • Document and escalate service issues • Monitor assigned clinicians’ work for adherence to standards and timelines • Support the clinician network in managing expectations • Identify efficiencies and solutions to issues • Liaise with clients/patients, insurers, employers and healthcare professionals • Act as the first point of contact for service providers and corporate customers • Help manage and track complex cases • Maintain databases and client/case files accurately and confidentially
• Bachelor’s degree in a related field or equivalent experience and training • Case Coordination and/or Case Management experience is a major asset • Expertise in Disability Treatment and Management principles, practices and processes • Proficiency with Microsoft Word, Excel and PowerPoint • Excellent verbal and written English communication skills • Effective self-directed work style with ability to work with minimal supervision • Strong administrative and customer service skills • Effective interpersonal skills and ability to communicate with varied stakeholders • Ability to manage multiple demands under time constraints and deadlines • Adaptability to changing priorities • Ability to contribute effectively to a positive work environment
