📋 Description

• Assess and evaluate members’ needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Facilitate member and family interaction with care resources • Manage members’ physical, environmental, and psychosocial health issues using varied strategies and techniques • Identify and resolve barriers to effective care • Continuously monitor patient care through assessments and evaluations • Ensure patients progress toward desired outcomes • Create member care plans when needed • Interpret department, segment, and organizational strategy and operating objectives • Make independent decisions regarding work methods, including in ambiguous situations • Follow established guidelines and procedures

🎯 Requirements

• LCSW, LPC, LAC, or LMFT • Experience working with Department of Children and Family Services • Applicable State licensure in field of study • Passion for contributing to an organization focused on continuously improving consumer experiences • Managed Care Experience (preferred) • Bilingual (preferred) • Must complete tuberculosis (TB) screening if selected • Minimum home internet download speed of 25 Mbps • Minimum home internet upload speed of 10 Mbps • Dedicated work space without ongoing interruptions to protect member PHI/HIPAA information
