📋 Description

• Provide assessment, ongoing treatment and management of biopsychosocial aspects of health and well-being for identified patients • Participate in the development, implementation, monitoring and evaluation of programs to meet patient needs • Assist health promotion, disease prevention, and chronic disease management in collaboration with healthcare team • Monitor and evaluate service delivery for individual clients, communities, and populations

🎯 Requirements

• Graduate of an accredited University • Registered and in good standing with a regulated health professional College • Minimum of five years clinical work experience • Extensive mental health clinical training • Skills/Abilities: Advanced level of mental health knowledge and experience working with adults, children, youth, & families • Knowledge of addictions care and navigation • Strong clinical skills and experience assessing and managing mental health presentations • Experience with developing and facilitating groups • Knowledge of relevant health care legislation and community mental health resources
